This article describes a multi-level analysis, which was carried out to explain the remuneration determinants of Director Generals (DGs) in Italian health authorities, using data drawn from the employees’ annual account for 2012. The investigation shows that salaries of top managers do not seem to significantly depend on legislative factors, as recommended by Prime Ministerial Decree (D.P.C.M.) 502/1995. Besides, a large amount of wage variability is explained by regional variances. Among the most relevant regional variables we find the obligation to submit to procedures of debt recovery, financial rebalancing or service reorganization.
